Neste artigo, apresentamos algumas experiências realizadas na disciplina de Estágio Supervisionado em Educação Infantil I, do curso de Pedagogia da UNIVATES, no qual investigou-se as possibilidades de conciliar a funcionalidade do espaço da sala de aula dos bebês com objetivos pedagógicos. Os dados foram obtidos mediante a realização de filmagens, fotografias e do uso de Diário de Campo. Observa-se que buscar contemplar na sala de aula de Educação Infantil os interesses e necessidades das crianças e dos adultos, exige dos docentes (...) uma escuta sensível do que as crianças manifestam, bem como, a criação de novos materiais e uso de diferentes estratégias a fim de garantir a multissensorialidade do espaço. Neste ensaio visual apresento um conjunto de imagens que se originam dos resíduos do meu processo de criação. A partir da produção de desenhos em grandes dimensões, que têm como tema elementos de uma natureza que brota nas cidades, na periferia do olhar, são gerados resíduos que surgem revelando outras camadas do meu processo criativo em desenho e resultam em reflexões acerca da linguagem se apresentando como objetos, ações, instalações ou obras bidimensionais. Deste modo, identifiquei um desenho que está à (...) margem do processo de desenhar, e que se coloca como um contorno das ações centrais, constituindo-se numa reflexão sobre a minha prática. Many sentences can be interpreted both as a metaphor and as a literal claim, depending on the context. The aim of this paper is to show that there are discourse-based systematic constraints on the identification of an utterance as metaphorical, literal, or both (as in the case of twice-apt metaphors), from a normative point of view. We claim that the key is contextual coherence. In order to substantiate this claim, we introduce a novel notion of context as a rich and (...) heterogeneous body of information, including previous discourse, elements coming from the surroundings of the utterance, background information, and Questions Under Discussion (QUD) issued from these three sources. We then define contextual coherence as a relation between what we call the minimal paraphrase of the metaphor and the context, and argue that for an interpretation to be coherent two conditions must be met. First, the minimal paraphrase must address some question in the QUD stack. Second, it must be externally consistent, i.e. consistent with the available contextual information. Finally, we argue that an approach based on contextual coherence is better suited to deal with twice-true and twice-apt metaphors than traditional approaches based on semantic deviance or pragmatic lack of fit. In this paper, we propose an account of metaphor identification on the basis of contextual coherence. In doing so, we build on previous work by Nicholas Asher and Alex Lascarides that appeals to rhetorical relations in order to explain discourse structure and the constraints on the interpretation of metaphor that follow from it. Applying this general idea to our problem, we will show that rhetorical relations are sometimes insufficient and sometimes inadequate for deciding whether a given utterance is a case (...) of metaphor. They are insufficient, since rhetorical relations fall short at times of providing a basis for disambiguating between literal and metaphorical interpretations. In such cases, contextual information other than previous discourse needs to enter the picture. To this effect, we bring the idea of external consistency into play. Beyond that, though, we will argue that rhetorical relations are sometimes inadequate to account for coherence, if conceived as relations among sentences only. The reason is that extra-linguistic elements of the situation in which the sentence is uttered may be crucial for getting at the preferred interpretation. To account for these cases, we allow rhetorical relations to connect both with previous discourse and with extra-linguistic situations. In our final refinement of the notion of contextual coherence, we forfeit any appeal to rhetorical relations in favour of Questions Under Discussion (QUD). We defend the view that this account does not only explain the same sort of cases. What is more, it solves the issue of metaphor identification in impoverished contexts. (shrink)

El aumento de los costos sanitarios y financieros de las enfermedades, las discapacidades y las disparidades respalda la aceleración mundial de los intereses y las inversiones en IA (inteligencia artificial) sanitaria para lograr soluciones sanitarias mejores, más baratas, más rápidas y justas a escala mundial y local. Sin embargo, no existe un consenso sobre la aplicación práctica de los principios de la IA responsable en diversos sectores, estados y sistemas de creencias de todo el mundo. Este estudio de prueba de (...) concepto utiliza el marco ético pluralista global (el Contrato Social Personalista) para proporcionar, por tanto, el primer análisis conocido de ética computacional (AiCE, por sus siglas en inglés) y política basado en IA aumentada bayesiana que integra análisis clínicos, de rentabilidad y de disparidades en la atención sanitaria con datos representativos a nivel nacional para estimar el costo global de las disparidades en la atención sanitaria en la colonoscopia (CS, por sus siglas en inglés) y el ahorro de la CS habilitada por IA para reducirlas. El estudio sugiere que revertir las disparidades raciales, en particular entre hispanos y asiáticos, puede ahorrar a los sistemas sanitarios estadounidenses 17.610 millones de dólares al año, con un ahorro potencial de 625,40 millones de dólares para los hispanos y 289 millones de dólares para los asiáticos en particular (con un ahorro similar para las comunidades vulnerables en países de ingresos medios y bajos). Los resultados anteriores respaldan el imperativo de ahorro de costos que supone la inversión estratégica y de capacitación en estas medidas impulsadas por la IA para mejorar los objetivos estratégicos de sostenibilidad, eficacia, eficiencia y equidad (SEEE) de la atención sanitaria. Estos resultados empíricos informan el argumento bioético global más amplio de las dimensiones gemelas de la dignidad y la seguridad humanas (arraigadas en el relato personalista, multicultural y metafísico de la persona como miembro de la familia humana global) para destacar el imperativo ético de la IA para optimizar el rendimiento del ecosistema sanitario digital global. Semejante fin instrumental es un medio decisivo para avanzar hacia el fin último del bien común, en el que se salvaguarda el bien individual de cada persona y en el que éste encuentra su realización trabajando hacia él. Digital literature is playing an increasingly important role in children's everyday lives and opening up new paths for family literacy and early childhood education. However, despite positive effects of electronic books and picture-book apps on vocabulary learning, early writing, or phonological awareness, research findings on early narrative skills are ambiguous. Particularly, there still is a research gap regarding how app materiality affects children's story understanding. Thus, based on the ViSAR model for picture-book app analysis and data stemming from 12 digital (...) reading dyads containing German monolingual 2- to 3-year-olds and their caregivers this study assessed the narrative potential of a commercial picture-book app and how this is used in interaction. Results of the media analysis showed that the app provides a high number of narrative animations. These animations could be used interactively to engage the child in the story. However, results of the interaction analysis showed that adult readers do not exploit this potential due to their strong concentration on operative prompts and instructions. Furthermore, an explorative analysis of the relation between adults' utterances and children's story comprehension provided preliminary indicators regarding how the length of reading duration and the number of utterances might relate to children's understanding of the story. Findings and methodological limitations of the study are discussed and combined didactically with practical recommendations on how to use narrative animations in interaction effectively. (shrink)

En 2011 el señor Mario Posada Ochoa donó a la Universidad EAFIT una extensa y poco conocida colección de arte; acaso unos cuantos amigos habrían visto de cerca las 480 obras que recibió la Universidad, de artistas como Segundo Angelvis, Inés Acevedo Bernal, Sergio Trujillo Magnenat, Luis A. Rengifo y Ramón Vásquez, entre otros. Algunas de ellas carecían de la firma del autor o de un dato que comprobara la autoría, y estaban deterioradas, más que por el tiempo, por el (...) improvisado bodegaje: don Mario las había guardado durante cuarenta años en su hermosa casa de Maracaibo, en el centro de la ciudad. States have been introduced on commutative and non-commutative algebras of fuzzy logics as functions defined on these algebras with values in [0,1]. Starting from the observation that in the definition of Bosbach states there intervenes the standard MV-algebra structure of [0,1], in this paper we introduce Bosbach states defined on residuated lattices with values in residuated lattices. We are led to two types of generalized Bosbach states, with distinct behaviours. This aim of this paper is two-fold: it critically analyses and rejects accounts blending active inference as theory of mind and enactivism; and it advances an enactivist-dynamic understanding of social cognition that is compatible with active inference. While some social cognition theories seemingly take an enactive perspective on social cognition, they explain it as the attribution of mental states to other people, by assuming representational structures, in line with the classic Theory of Mind. Holding both enactivism and ToM, we argue, (...) entails contradiction and confusion due to two ToM assumptions widely known to be rejected by enactivism: that social cognition reduces to mental representation and social cognition is a hardwired contentful ‘toolkit’ or ‘starter pack’ that fuels the model-like theorising supposed in. The paper offers a positive alternative, one that avoids contradictions or confusion. After rejecting ToM-inspired theories of social cognition and clarifying the profile of social cognition under enactivism, that is without assumptions and, the last section advances an enactivist-dynamic model of cognition as dynamic, real-time, fluid, contextual social action, where we use the formalisms of dynamical systems theory to explain the origins of socio-cognitive novelty in developmental change and active inference as a tool to demonstrate social understanding as generalised synchronisation. Background In the UK, the solidaristic character of the NHS makes it one of the most trusted public institutions. In recent years, the introduction of data-driven technologies in healthcare has opened up the space for collaborations with private digital companies seeking access to patient data. However, these collaborations appear to challenge the public’s trust in the. Main text In this paper we explore how the opening of the healthcare sector to private digital companies challenges the existing social contract and the (...) NHS’s solidaristic character, and impacts on public trust. We start by critically discussing different examples of partnerships between the NHS and private companies that collect and use data. We then analyse the relationship between trust and solidarity, and investigate how this relationship changes in the context of digital companies entering the healthcare system. Finally, we show ways for the NHS to maintain public trust by putting in place a solidarity grounded partnership model with companies seeking to access patient data. Such a model would need to serve collective interests through, for example, securing preferential access to goods and services, providing health benefits, and monitoring data access. Conclusion A solidarity grounded partnership model will help establish a social contract or licence that responds to the public’s expectations and to principles of a solidaristic healthcare system. Pragmatic comparative effectiveness randomized controlled trials evaluate the effectiveness of one interventions under real-world clinical conditions. The results of ceRCTs are often directly generalizable to everyday clinical practice, providing information critical to decision-making by patients, clinicians, and healthcare policymakers. The PRECIS-2 framework identifies nine domains that serve to score a trial on a continuum between very explanatory to very pragmatic. On 26 February 2020, the German Constitutional Court rejected a law from 2015 that prohibited any form of ‘business-like’ assisted suicide as unconstitutional. The landmark ruling of the highest federal court emphasised the high priority given to the rights of autonomy and free personal development, both of which constitute the principle of human dignity, the first principle of the German constitution. The ruling echoes particularities of post-war Germany’s end-of-life debate focusing on patient self-determination while rejecting any discussion of active assistance (...) to die through a lethal injection administered by a doctor. O artigo apresenta resultados iniciais de uma investigação sobre o modo como os jovens pesquisadores do Conicet utilizam e vivenciam o tempo em suas práticas cotidianas. O ponto de partida é que a inserção de lógicas competitivas de avaliação e acreditação tem como efeito o aumento dos níveis de produtividade necessários para alcançar e sustentar postos de trabalho, o que em termos temporais se traduz numa racionalização da equação tempo/resultados e em diferentes formas, ambas objetivas e subjetivo, da pressão do (...) tempo. Como o fenômeno não foi estudado anteriormente na Argentina, a pesquisa tem caráter exploratório e se limita ao grupo potencialmente mais exposto a essa dinâmica, o dos jovens em início de carreira. O principal resultado é que esses jovens precisam enfrentar e resolver quatro grandes tensões temporárias: aquela que contrasta o trabalho com outros aspectos da vida; aquela que contrasta as demandas de longo prazo da graduação com as da agenda diária ou de curto prazo; aquela que surge das exigências de diferentes aspectos do trabalho; aquele que opõe o interesse de fazer bem à necessidade de fazer rapidamente. Após apresentar sua abordagem teórico-metodológica, o artigo expõe as diferentes formas como essas tensões são vivenciadas e gerenciadas. (shrink)

In this paper we propose a narrative account for the origin of language. Such a proposal is based on two assumptions. The first is conceptual and concerns the idea that the distinctive feature of human language (what sets it apart from other forms of animal communication) has to be traced to its inherently narrative character. The second assumption is methodological and connected to the idea that the study of language origin is closely related to the analysis of the cognitive systems (...) at the base of narrative. Research on narrative abilities of subjects with Autism Spectrum Disorder has shown that storytelling requires the capability to link events causally connected to one another, and especially events which are remote from one another on the temporal axis of a story. Based on this research, we hypothesize that an important cognitive device involved in narrative is Mental Time Travel (MTT), that is, the system that allows humans to project themselves into the past and future. We show that such a system is present (to a greater or lesser extent) even in non-human animals. By virtue of this, we argue that MTT is independent of language and that it may be considered a cognitive precursor for the origin of language. Specifically, we propose that MTT allowed our ancestors to develop a form of pantomimic communication that might be considered as the foundation of the narrative origin of language. (shrink)

On July 2021, the UK High Court of Justice heard the Case CO/2066/2020 on the application of Heidi Crowter who lives with Down’s syndrome, and Máire Lea-Wilson whose son Aidan has Down’s syndrome. Crowter and Lea-Wilson, with the support of the disability rights campaign, ‘Don’t Screen Us Out’, have been taking legal action against the Secretary of State for Health and Social Care (the UK Government) for a review of the 1967 Abortion Act: the removal of section 1(1)(d) making termination (...) of pregnancy lawful for ‘severe’ fetal indications detected after 24 weeks' gestation. On 23 September 2021, the High Court dismissed the claim. This action came at a time when non-invasive prenatal testing (NIPT) was introduced into the NHS England Fetal Anomaly Screening Programme for the trisomies 21, 13 and 18. The implementation of NIPT has been heavily criticised, in particular by ‘Don’t Screen Us Out’ campaigners, for increasing fetal selection and discrimination of people living with disabilities. The case of Crowter and Lea-Wilson echoes debates in other European countries such as in France and Germany, where the introduction of NIPT in the public healthcare system has provoked equally vehement public reactions and discussions. The comparison between these three countries allows contextualising the public discourses around NIPT and the ground for termination of pregnancy in relation to different socio-cultural and political contexts. We examine how each country, and particularly England, deals with the conflict between the principles of promoting the rights of people living with disabilities and preserving women’s reproductive autonomy. This chapter takes inspiration from Wittgenstein’s thinking to formulate a non-reductive toolbox for the study of religion associated with generative modelling, specifically as applied in complex adaptive systems theory. It converges on a communal perspective on religion as multiscale active inference that contrasts starkly with common ‘straw person’ perspectives on religion that reduce it to ‘erroneous’ theorising generated by the brain. In contrast, we argue, religious practices at the enculturated level of description involve implicit and explicit meanings, experienced both individually (...) and collectively. Firstly, we characterise these in terms of Wittgenstein’s notions of rule-following and language games. Secondly, we discuss how the collective and perspectival aspects of religion in enculturation with morals, doctrines, rituals, and expressions can be formalised in terms of deep active inference in multi-agent systems. This approach describes how religions are incommensurable with scientific methods due to the epistemic separation between the different kinds of language games of religious and scientific practices. Their touchpoints tend to give rise to perpetual confusion and unproductive discussions, requiring perspicuous presentations that can be formulated with our toolbox. In this sense, our account has implications for tackling complex societal challenges such as ethical policy-making. (shrink)

In this short response, I agree with Cavaliere’s recent invitation to consider ectogenesis, the process of gestation occurring outside the body, as a political perspective and provocation to building a world in which reproductive and care labour are more justly distributed. But I argue that much of the literature Cavaliere addresses in which scholars argue that artificial wombs may produce greater gender equality has the limitation of taking a fixed, binary and biological approach to sex and gender. I argue that (...) in taking steps toward the possibility of more just practices of caregiving and family making, we must look first not to artificial womb technologies but to addressing the ways that contemporary legal and social practices that enforce essentialising, binary ways of thinking about reproductive bodies inhibit this goal. This paper presents the results of a qualitative interview study exploring English and French physicians’ moral perspectives and attitudes towards end-of-life decisions when patients lack capacity to make decisions for themselves. The paper aims to examine the importance physicians from different contexts accord to patient preferences and to explore the role of advance directives in each context. The interviews focus on problems that emerge when deciding to withdraw/-hold life-sustaining treatment from both conscious and unconscious patients; decision-making procedures and the participation (...) of proxies/relatives; previous experience with ADs and views on their usefulness; and perspectives on ways in which the decision-making processes in question might be improved. The analysis reveals differences in the way patient preferences are taken into consideration and shows how these differences influence the reasons physicians in each country invoke to justify their reluctance to adhering to ADs. Identifying cultural differences that complicate efforts to develop the practical implementation of ADs can help to inform national policies governing ADs and to better adapt them to practice. Trenton Merricks has recently argued that substance dualist accounts of embodiment and humanness do not cohere well with the Incarnation. He has also claimed that physicalism about human persons avoids this problem, which should lead Christians to be physicalists. In this paper, I argue that there are plausible dualist accounts of embodiment and humanness that avoid his objections. Furthermore, I argue that physicalism is inconsistent with the Incarnation.

The rising demographic of older adults worldwide has led to an increase in dementia cases. In order to ensure the proper allocation of care and resources to this clinical group, it is necessary to correctly distinguish between simulated versus bona-fide cognitive deficits typical of dementia. Performance Validity Tests are specifically designed to assess a lack of effort and the possible simulation of cognitive impairment. Previous research demonstrates that PVTs may be sensitive to dementia, thus inaccurately classifying real memory impairment as (...) simulation. Here, we analyzed the sensitivity of PVTs in discriminating between dementia and simulation using receiver operating characteristic curve analyses. Further, we examined the potential need for adjusting cut-off scores for three stand-alone and one embedded PVT for Portuguese older adults with dementia. The results showed that all measures, except for the Coin in Hand— Extended version, were sensitive to one or more sociodemographic and/or cognitive variables, and it was necessary to adjust cut-off points for all measures. Additionally, the Rey-15 Item Memory Test did not demonstrate sufficient discriminating capacity for dementia. These results present important implications for clinical practice and the daily life of patients, as the use of incorrect cut-off points could impede patients from getting the resources they need. In Belgium, a service voucher scheme – known as Titres Services – was launched in 2004 in order to create employment and regularize the labor conditions of domestic workers. The extent to which this scheme has represented an improvement in domestic workers’ labor conditions, however, is still a matter of debate. This article explores the workers’ experience of the changes introduced by this scheme. It focuses on Latin American migrants that are currently working under this scheme in Brussels, situating them (...) in relation to their previous experiences and the experience of other migrants who currently work in the informal market. The authors distinguish two tropes in their informants’ discourse, which describe their ambivalence regarding these changes. Using the Titres Services scheme’s rhetoric, the first one seeks to increase the social status of this occupation by presenting it as a ‘profession’. Contrarily, the second trope highlights the limits to professionalization. (shrink)

Young people around the world show considerable engagement with climate change. How can education draw on this engagement in order to benefit students and society? In this article, we discuss how inquiry-based dialogues can support students’ development in their societal engagement. We argue that such dialogues should include real-world problems involving disagreement, which promote students’ agency. We elaborate on qualities of dialogues, such as developing argumentation and perspectives together through respect, attentive listening and recognition of others’ viewpoints. Central theoretical perspectives (...) include inquiry-based dialogues and students’ lived democracy. This research draws on data from a classroom activity where 15–16-year-old students in Norway investigate controversial issues related to electrifying offshore oil platforms. The students are in a learning situation, and the teacher wants them to learn about climate change and the oil industry and the skills and attitudes needed to participate in inquiry-based dialogues. The dialogues show that students bring knowledge, opinions and attitudes from outside school, characteristics that are crucial for students’ engagement. We discuss how teachers can facilitate students’ further engagement by addressing multiple aims of inquiry-based dialogues. (shrink)

According to Hartry Field, the mathematical Platonist is hostage of a dilemma. Faced with the request of explaining the mathematicians’ reliability, one option could be to maintain that the mathematicians are reliably responsive to a realm populated with mathematical entities; alternatively, one might try to contend that the mathematical realm conceptually depends on, and for this reason is reliably reflected by, the mathematicians’ (best) opinions; however, both alternatives are actually unavailable to the Platonist: the first one because it is in (...) tension with the idea that mathematical entities are causally ineffective, the second one because it is in tension with the suggestion that mathematical entities are mind-independent. John Divers and Alexander Miller have tried to reject the conclusion of this argument—according to which Platonism is inconsistent with a satisfactory epistemology for arithmetic—by redescribing the second horn of the dilemma in light of Crispin Wright’s notion of judgment-dependent truth; in particular they have contended that once arithmetical truth is conceived in this way the Platonist can have a substantial epistemology which does not conflict with the idea that the mathematical entities exist mind-independently. In this paper I analyze Wright’s notion of judgment-dependent truth, and reject Divers and Miller’s argument for the conclusion that arithmetical truth can be so characterized. In the final part, I address the worry that my argument generalizes very quickly to the conclusion that no area of discourse could be characterized as judgment-dependent. As against this conclusion, I indicate under what conditions—notably not satisfied in Divers and Miller’s case, but possibly satisfied in others—a discourse’s judgment-dependency can be successfully vindicated. In spite of the intuitiveness of epistemic closure, there has been a stubborn stalemate regarding whether it is true, largely because some of the “Moorean” things we seem to know easily seem clearly to entail “heavyweight” philosophical things that we apparently cannot know easily—or perhaps even at all. In this paper, I will show that two widely accepted facts about what we do and don’t know—facts with which any minimally acceptable understanding of knowledge must comport—are jointly inconsistent with the truth (...) of CLR. The proof works by supposing the truth of “Categorialism,” a thesis about the relation between basic categories and common nouns and predicates, which is itself a heavyweight claim that cannot be easily known to be either true or false. If this perspective were to hold true, then humans could not be held responsible for the actions of autonomous AI systems.If we were to conceive of AI as an ideal entity that exists independently of humans, it would likely lead to a number of negative consequences for society. First and foremost, it may lead to the dangerous assumption that AI systems are neutral and objective, devoid of the biases and prejudices that are inherent in human decision-making. This could create a false sense of trust in AI, leading to the belief that their decisions are inherently fair and impartial. However, the reality is that AI systems are not inherently neutral, but rather are shaped by the cultural and social contexts in which they are created and deployed. Lepore and Stone seek to replace the rationality-based Gricean picture of coordination between speaker and hearer with one leaning more strongly on the roles of convention and speaker knowledge while doing away with conversational implicature. Focusing on the phenomena of indirect speech acts, asymmetric conjunction, and scalar inferencing, I argue that the case for abandoning implicature as an analytical tool is not ultimately compelling. I seek further to demonstrate the utility of the classical Gricean distinction between what is said and (...) what is implicated in the construction of an intuitively appealing approach to the perennial question of how to distinguish lying from misleading inside and outside the legal domain. This article examines cases that illustrate the relation of information structure to truth-conditional semantics, grammatical form, and assertoric force. Before discussing the interaction between information structure and at-issue meaning, it considers the nature of information and what constitutes information. It then looks at two aspects of the common ground, common ground content and CG management, as well as the criteria of category membership. The article also explores the varying degrees of at-issueness, the role of rhetorical opposition and but clauses, as (...) well as the variable strength of at-issue content. The landscape of non-at-issue meaning is presented, and the distinction between conventional implicature and assertorically inert entailments is highlighted using a range of distributional diagnostics. This document exposes the conflict from the Post-Rational cognitive perspective, understanding the conflict as a relational phenomenon, which emerges when the need of recognition is exposed to its contrary: the non-recognition. “To know oneself” means in fact, to recognize oneself through the mediation of the other. An individual develops himself by recognizing the “otherness” that constitutes him. The self that goes out toward the other and then returns as ipse/selfhood, having acquired self-awareness through the other. For this reason, recognition is (...) a fundamental element in the building of human subjectivity. Identity is a continuous process that lasts a lifetime. It contains a central nucleus that ensures continuity by which a person is able to recognize him/ herself. In this way individuals learn to define who they are through interactions with others, especially those whom are more significant. Being recognized by others and by the environment offer the individual a sense of unity and coherence about himself. Conflict arises when the other or the environment becomes a threat to the survival of the subjectivity; when the absence or lack of recognition leads to harm in a person. Relational experiences are able to engender and ensure recognition, while holding onto an indispensable acceptance of individual differences. The ethics of the Flexibility In duty hour Requirements for Surgical Trainees trial have been vehemently debated. Views on the ethics of the FIRST trial range from it being completely unethical to wholly unproblematic. The FIRST trial illustrates the complex ethical challenges posed by cluster randomised trials of policy interventions involving healthcare professionals. In what follows, we have three objectives. First, we critically review the FIRST trial controversy, finding that commentators have failed to sufficiently identify and address many of the (...) relevant ethical issues. The 2012 Ottawa Statement on the Ethical Design and Conduct of Cluster Randomized Trials provides researchers and research ethics committees with specific guidance for the ethical design and conduct of CRTs. Second, we aim to demonstrate how the Ottawa Statement provides much-needed clarity to the ethical issues in the FIRST trial, including: research participant identification; consent requirements; gatekeeper roles; benefit-harm analysis and identification of vulnerable participants. We nonetheless also find that the FIRST trial raises ethical issues not adequately addressed by the Ottawa Statement. Hence, third and finally, we raise important questions requiring further ethical analysis and guidance, including: Does clinical equipoise apply to policy interventions with little or no evidence-base? Do healthcare providers have an obligation to participate in research? Does the power-differential in certain healthcare settings render healthcare providers vulnerable to duress and coercion to participant in research? If so, what safeguards might be implemented to protect providers, while allowing important research to proceed? (shrink)
